Thank you for booking a Separation Anxiety service with Bespoke Behaviours. Please complete the below forms as fully as you can to provide valuable information about your dog so that we are aware of as much about your dog as possible before we even meet.

We appreciate that the forms are very detailed but this will allow us to work together most effectively and efficiently.

Please be honest with the information you provide. There is no judgement whatsoever, we just need the full picture to understand your dog better and to establish the most appropriate way to work with them. If you don't know an answer to a question, please say so.

If a question is not relevant to your dog, please leave it blank or put N/A.

Separation Anxiety Owners Observations

All dogs primarily use body language to communicate with each other and with us, their caregivers.

It is really important that we learn to be experts in reading our own dog’s body language. In this way we can spot signs of anxiety and make sure that we always keep the dog feeling ‘safe’ and under their threshold. We never want the dog to be feeling anxious as this can set our training back.

To become the expert in your dog’s body language, you will need to spend time watching and noting down all those little things that they do in different states of arousal. For example, look at where they hold their tail, what position their ears are in, is their mouth open / closed, are their eyes wide or relaxed, are they pacing around, are they sniffing at stuff, what position are they in etc. Basically, become a detective and watch for all those obvious and micro behaviours that are happening in different situations.

When you become familiar with their little quirks and tell-tale signs, you will become that expert in recognising your dog’s stress levels and being able to support them accordingly.

In order to fill in this form, you may wish to spend some time observing your dog and come back to fill it in later. Please ensure it is submitted at least 48 hours before your consultation.

Please note your dogs body language in the following states:
